To describe a novel method of measuring the degree of ophthalmic viscosurgical device (OVD) penetration into intraocular tissues using a combination of polarized light imaging and a linear polarized light source during vitreoretinal surgery. The OVD, which was used for fluid-air exchange during a two-port pars plana vitrectomy (PPV) procedure, was imaged at various times in the surgical procedure using a system that generates high-quality images of birefringence in a linear polarization state. The center of the images is either in the fovea or near the equator, depending on the amount of OVD used. Three to five images were taken from each patient's eye using multiple device settings. OVD penetration into the tissues of the eye was quantified from the polarized light images. A hand-held, compact, portable imaging system using linear polarized light was used. Fourteen patients with various indications for vitrectomy participated in the study. The degree of OVD penetration into the tissues of the eye was correlated with the amount of OVD used. A statistically significant relationship between the amount of OVD used and the degree of OVD penetration into the eye was observed. The extent of OVD penetration into the eye was related to the amount of OVD used in the vitrectomy procedure. When OVD penetration is excessive, the fluid-air exchange procedure must be timed to optimize the fluid flow into the vitreous cavity. Polarized light imaging can be used to determine the degree of OVD penetration into the eye and guide the surgical procedure.Q:
